GAS ATHLETIC KICKBOXING & BJJ LIABILITY WAIVER AND RELEASE

Participant Agreement, Release of Liability, and Assumption of Risk

1. ACKNOWLEDGMENT OF RISKS I, {name}, understand that participation in kickboxing, Brazilian Jiu-Jitsu (BJJ), and related physical fitness activities at Gas Athletic Kickboxing & BJJ ("Gym") involves inherent risks, including but not limited to physical exertion, falls, collisions, equipment-related injuries, muscle strain, fractures, and in rare cases, serious injury or death.

I acknowledge that participation in these activities is voluntary, and I assume full responsibility for any risks of injury, illness, or damage associated with my participation.

2. RELEASE OF LIABILITY & WAIVER OF CLAIMS In consideration of being allowed to participate in training, classes, open gym, sparring, and other activities at the Gym, I release, waive, discharge, and hold harmless Gas Athletic Kickboxing & BJJ, its owners, employees, instructors, trainers, affiliates, and agents from any liability, claims, or causes of action arising from or related to any injury, disability, death, property damage, or other loss, including injuries resulting from negligence, to the fullest extent permitted by Arizona law.

3. ASSUMPTION OF RISK I acknowledge and assume all risks associated with training and engaging in physical activity at the Gym.

I understand that the Gym does not provide medical insurance or coverage and that I am solely responsible for any medical expenses in the event of an injury.

4. MEDICAL CONSENT & EMERGENCY CARE I authorize the Gym’s employees, instructors, and staff to obtain emergency medical care on my behalf if deemed necessary, and I agree to bear all costs associated with such care.

I understand that the Gym is not responsible for any medical expenses or related costs.

5. RULES, SAFETY, & CONDUCT I agree to follow all Gym rules, policies, and safety instructions, including proper equipment use, hygiene standards, and respectful conduct towards instructors and other participants.

I acknowledge that failure to comply with Gym policies may result in my removal from the premises without refund.

6. CONSENT TO USE IMAGE & LIKENESS I grant permission to the Gym to use photographs, video recordings, or other media containing my likeness for promotional, marketing, or educational purposes, without compensation.

If I wish to opt out, I will notify the Gym in writing.

7. MINOR PARTICIPANTS (IF APPLICABLE) If the participant is under the age of 18, a parent or legal guardian must complete this waiver on behalf of the minor.

As the parent/guardian, I have read and understood this waiver and agree to its terms on behalf of my child.

8. GOVERNING LAW & DISPUTE RESOLUTION I agree that this agreement shall be governed by the laws of the State of Arizona.

I agree that any disputes arising from this agreement shall be resolved through mediation or arbitration before litigation, to the fullest extent permitted by law.

9. SEVERABILITY CLAUSE I understand that if any provision of this agreement is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.

ACKNOWLEDGMENT & ACCEPTANCE I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS TERMS.

I ACKNOWLEDGE THAT BY CHECKING THESE BOXES, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.
